switch_region_table_read() accesses the region table with READ_ONCE()
and is called from the lockless switch_map() IO path. However,
switch_region_table_write() stores to the same array with a plain
assignment. This results in an inconsistent access pattern for a
lockless shared variable and may trigger data race reports.

Use WRITE_ONCE() to pair with the existing READ_ONCE() in
switch_region_table_read().

This is the exact same thing as the 'alloc_obj()' version, only much
smaller because there are a lot fewer users of the *alloc_flex()
interface.

As with alloc_obj() version, this was done entirely with mindless brute
force, using the same script, except using 'flex' in the pattern rather
than 'objs*'.

This is the result of running the Coccinelle script from
scripts/coccinelle/api/kmalloc_objs.cocci. The script is designed to
avoid scalar types (which need careful case-by-case checking), and
instead replace kmalloc-family calls that allocate struct or union
object instances:

Single allocations:	kmalloc(sizeof(TYPE), ...)
are replaced with:	kmalloc_obj(TYPE, ...)

Array allocations:	kmalloc_array(COUNT, sizeof(TYPE), ...)
are replaced with:	kmalloc_objs(TYPE, COUNT, ...)

Flex array allocations:	kmalloc(struct_size(PTR, FAM, COUNT), ...)
are replaced with:	kmalloc_flex(*PTR, FAM, COUNT, ...)

(where TYPE may also be *VAR)

The resulting allocations no longer return "void *", instead returning
"TYPE *".

This adds a 'bool *forward' parameter to .prepare_ioctl, which allows
device mapper targets to accept ioctls to themselves instead of the
underlying device. If the target already fully handled the ioctl, it
sets *forward to false and device mapper won't forward it to the
underlying device any more.

In order for targets to actually know what the ioctl is about and how to
handle it, pass also cmd and arg.

As long as targets restrict themselves to interpreting ioctls of type
DM_IOCTL, this is a backwards compatible change because previously, any
such ioctl would have been passed down through all device mapper layers
until it reached a device that can't understand the ioctl and would
return an error.

Eliminate duplicate boilerplate code for simple modules that contain
a single DM target driver without any additional setup code.

Add a new module_dm() macro, which replaces the module_init() and
module_exit() with template functions that call dm_register_target()
and dm_unregister_target() respectively.
