[media] include/media: move driver interface headers to a separate dir

Let's not mix headers used by the core with those headers that are needed by some driver-specific interface header. [media] saa7146: remove V4L2_FL_LOCK_ALL_FOPS

Add proper locking to the file operations, allowing for the removal of the V4L2_FL_LOCK_ALL_FOPS flag. I also removed some dead code in the form of the saa7146_devices list and saa7146_devices_lock mutex: these were used once but that was a long time ago. [media] saa7146: fix querycap, vbi/video separation and g/s_register

The querycap ioctl returned an incorrect version number and incorrect capabilities (mixing up vbi and video caps). The reason for that was that video nodes could do vbi activities: that should be separated between the vbi and video nodes. There were also a few minor problems with dbg_g/s_register that have been resolved. The mxb/saa7146 driver now passes the v4l2_compliance tests. include: replace linux/module.h with "struct module" wherever possible

The pretty much brings in the kitchen sink along with it, so it should be avoided wherever reasonably possible in terms of being included from other commonly used files, as it results in a measureable increase on compile times. The worst culprit was probably device.h since it is used everywhere. This file also had an implicit dependency/usage of mutex.h which was masked by module.h, and is also fixed here at the same time. There are over a dozen other headers that simply declare the struct instead of pulling in the whole file, so follow their lead and simply make it a few more. Most of the implicit dependencies on module.h being present by these headers pulling it in have been now weeded out, so we can finally make this change with hopefully minimal breakage. Standardize the mechanisms to emit logging messages. A few other modules used an #include from saa7146, convert those at the same time. Add pr_fmt. Convert printks to pr_ Convert printks without KERN_ to appropriate pr_. Convert logging macros requiring multiple parentheses to normal style. Removed embedded prefixes when pr_fmt was added. Whitespace cleanups when around other conversions. Use printf extension %pM to print mac address. Coalesce format strings. With all the v4l2_subdev changes that were made to these drivers it is a good idea to increase the version number of each driver. It's just the patch level that is increased, except for the zoran and saa7146 drivers where the minor number was increased due to the more substantial changes that were made to those two drivers.
