Replace the #include of &lt;linux/mod_devicetable.h&gt; by the more specific
&lt;linux/device-id/*.h&gt; where applicable. For most cases the include
can be dropped completely, only a few drivers need one or two headers
added.

The ephy control register on bcm63xx SoCs contain bits for
resetting individual internal phys. Use the existing bcm6345-reset
driver to set bits.

This function call was found to be unnecessary as there is no equivalent
platform_get_drvdata() call to access the private data of the driver. Also,
the private data is defined in this driver, so there is no risk of it being
accessed outside of this driver file.
