The documentation is in a format that is very close to ReST format.

The conversion is actually:
  - add blank lines in order to identify paragraphs;
  - fixing tables markups;
  - adding some lists markups;
  - marking literal blocks;
  - adjust some title markups.

At its new index.rst, let's add a :orphan: while this is not linked to
the main index.rst file, in order to avoid build warnings.

We're now ready to map our vectors in weird and wonderful locations.
On enabling ARM64_HARDEN_EL2_VECTORS, a vector slot gets allocated
if this hasn't been already done via ARM64_HARDEN_BRANCH_PREDICTOR
and gets mapped outside of the normal RAM region, next to the
idmap.

That way, being able to obtain VBAR_EL2 doesn't reveal the mapping
of the rest of the hypervisor code.

The Documentation/arm64/memory.txt says:
When using KVM, the hypervisor maps kernel pages in EL2, at a fixed
offset from the kernel VA (top 24bits of the kernel VA set to zero):

In fact, kernel addresses are transleted to HYP with kern_hyp_va macro,
which has more options, and none of them assumes clearing of top 24bits
of the kernel VA.

This patch allows support for 3 levels of page tables with 64KB page
configuration allowing 48-bit VA space. The pgd is no longer a full
PAGE_SIZE (PTRS_PER_PGD is 64) and (swapper|idmap)_pg_dir are not fully
populated (pgd_alloc falls back to kzalloc).

Rather than guessing what the maximum vmmemap space should be, this
patch allows the calculation based on the VA_BITS and sizeof(struct
page). The vmalloc space extends to the beginning of the vmemmap space.

Since the virtual kernel memory layout now depends on the build
configuration, this patch removes the detailed description in
Documentation/arm64/memory.txt in favour of information printed during
kernel booting.

Add support for early IO or memory mappings which are needed before the
normal ioremap() is usable.  This also adds fixmap support for permanent
fixed mappings such as that used by the earlyprintk device register
region.
