<title>docs: move other kAPI documents to core-api</title>

There are a number of random documents that seem to be
describing some aspects of the core-api. Move them to such
directory, adding them at the core-api/index.rst file.

<title>bus-virt-phys-mapping.txt: standardize document format</title>

Each text file under Documentation follows a different
format. Some doesn't even have titles!

Change its representation to follow the adopted standard,
using ReST markups for it to be parseable by Sphinx:

- Move author info to the beginning of file and use :Author:
- use warning/note annotation;
- mark literal blocks as such;
- Add a title for the document;
- use **emphasis** instead of _emphasis_.

<title>doc: fix broken references</title>

There are numerous broken references to Documentation files (in other
Documentation files, in comments, etc.). These broken references are
caused by typo's in the references, and by renames or removals of the
Documentation files. Some broken references are simply odd.

Fix these broken references, sometimes by dropping the irrelevant text
they were part of.

Having both IO-mapping.txt and io-mapping.txt in Documentation/
was confusing and/or bothersome to some people, so rename
IO-mapping.txt to bus-virt-phys-mapping.txt.  Also update
Documentation/00-INDEX for both of these files.
