docs: ia64: convert to ReST

Rename the ia64 documentation files to ReST, add an index for them and adjust in order to produce a nice html output via the Sphinx build system. There are two upper case file names. Rename them to lower case, as we're working to avoid upper case file names at Documentation. At its new index.rst, let's add a :orphan: while this is not linked to the main index.rst file, in order to avoid build warnings. Signed-off-by: Mauro Carvalho Chehab

I got a "sched_setaffinity:: Invalid argument" error when using err_injection_tool to inject error on a system with over 32 cpus. Most of these fixes were already submitted for old kernel versions, and were approved, but for some reason they never made it into the releases. [IA64] Itanium MC Error Injection Tool: Doc and sample application

This patch contains a documention and sample application. Since the sample application has ~1000 lines of code, it might not be suitable in a kernel documention in kenrel tree.
