Nullify irqfd-&gt;producer if updating the IRTE for bypass fails, as leaving a
dangling pointer will result in a use-after-free if the irqfd is reachable
through KVM's routing, but the producer is freed separately.  E.g. for VFIO
PCI, the producer is embedded in struct "vfio_pci_irq_ctx" and freed when
the vector is disabled, which can happen independent of routing updates.

When userspace issues the KVM_SET_IRQCHIP ioctl to set the state of
the PIC, kvm_vm_ioctl_set_irqchip() grabs @kvm-&gt;arch.vpic-&gt;lock before
updating the state. However, the KVM_GET_IRQCHIP ioctl to retrieve the
same PIC state does not grab such lock, potentially causing torn reads
for userspace.

Fix this by grabbing the lock on the read path.

This issue goes all the way back. The bug was introduced with the
addition of PIC ioctl code itself in 6ceb9d791eee ("KVM: Add get/
set irqchip ioctls for in-kernel PIC live migration support"). Later,
894a9c5543ab ("KVM: x86: missing locking in PIT/IRQCHIP/SET_BSP_CPU
ioctl paths") added the locking for kvm_vm_ioctl_set_irqchip(), but
missed kvm_vm_ioctl_get_irqchip().

KVM x86 APIC-ish changes for 6.20

 - Fix a benign bug where KVM could use the wrong memslots (ignored SMM) when
   creating a vCPU-specific mapping of guest memory.

 - Clean up KVM's handling of marking mapped vCPU pages dirty.

 - Drop a pile of *ancient* sanity checks hidden behind in KVM's unused
   ASSERT() macro, most of which could be trivially triggered by the guest
   and/or user, and all of which were useless.

 - Fold "struct dest_map" into its sole user, "struct rtc_status", to make it
   more obvious what the weird parameter is used for, and to allow burying the
   RTC shenanigans behind CONFIG_KVM_IOAPIC=y.

 - Bury all of ioapic.h and KVM_IRQCHIP_KERNEL behind CONFIG_KVM_IOAPIC=y.

 - Add a regression test for recent APICv update fixes.

 - Rework KVM's handling of VMCS updates while L2 is active to temporarily
   switch to vmcs01 instead of deferring the update until the next nested
   VM-Exit.  The deferred updates approach directly contributed to several
   bugs, was proving to be a maintenance burden due to the difficulty in
   auditing the correctness of deferred updates, and was polluting
   "struct nested_vmx" with a growing pile of booleans.

 - Handle "hardware APIC ISR", a.k.a. SVI, updates in kvm_apic_update_apicv()
   to consolidate the updates, and to co-locate SVI updates with the updates
   for KVM's own cache of ISR information.

 - Drop a dead function declaration.

When disconnecting a non-MSI irqfd from an IRQ bypass producer, WARN if
the irqfd is configured for IRQ bypass and set its IRTE back to remapped
mode to harden against kernel/KVM bugs (keeping the irqfd in bypass mode
is often fatal to the host).

Deactivating an irqfd (removing it from the list of irqfds), updating
irqfd routes, and the code in question are all mutually exclusive (all
run under irqfds.lock).  If an irqfd is configured for bypass, and the
irqfd is deassigned at the same time IRQ routing is updated (to change the
routing to non-MSI), then either kvm_arch_update_irqfd_routing() should
process the irqfd routing change and put the IRTE into remapped mode
(routing update "wins"), or kvm_arch_irq_bypass_del_producer() should see
the MSI routing info (deactivation "wins").

Rework almost all of KVM x86's exports to expose symbols only to KVM's
vendor modules, i.e. to kvm-{amd,intel}.ko.  Keep the generic exports that
are guarded by CONFIG_KVM_EXTERNAL_WRITE_TRACKING=y, as they're explicitly
designed/intended for external usage.

Move kvm_intr_is_single_vcpu() to lapic.c, drop its export, and make its
"fast" helper local to lapic.c.  kvm_intr_is_single_vcpu() is only usable
if the local APIC is in-kernel, i.e. it most definitely belongs in the
local APIC code.
