Move all the MDIO drivers and multiplexers into drivers/net/mdio.  The
mdio core is however left in the phy directory, due to mutual
dependencies between the MDIO core and the PHY core.

Take this opportunity to sort the Kconfig based on the menuconfig
strings, and move the multiplexers to the end with a separating
comment.

Displaying the virtual address at which the MDIO base register address
has been mapped is not useful and is not visible with pointer hashing in
place, replace the message with something indicating successful
registration instead.

The mii management register in iproc mdio block
does not have a retention register so it is lost on suspend.
Save and restore value of register while resuming from suspend.

USB PHYs need the MDIO clock divisor enabled earlier to work.
Initialize mdio clock divisor in probe function. The ext bus
bit available in the same register will be used by mdio mux
to enable external mdio.

This patch adds support for the Broadcom iProc MDIO bus interface.
The MDIO interface can be found in the Broadcom iProc family Soc's.

The MDIO bus is accessed using a combination of command and data
registers. This MDIO driver provides access to the Etherent GPHY's
connected to the MDIO bus.
