<title>video: move fbdev to drivers/video/fbdev</title>

The drivers/video directory is a mess. It contains generic video related
files, directories for backlight, console, linux logo, lots of fbdev
device drivers, fbdev framework files.

Make some order into the chaos by creating drivers/video/fbdev
directory, and move all fbdev related files there.

No functionality is changed, although I guess it is possible that some
subtle Makefile build order related issue could be created by this
patch.

CONFIG_HOTPLUG is going away as an option.  As a result, the __dev*
markings need to be removed.

This change removes the use of __devinit, __devexit_p, __devinitdata,
__devinitconst, and __devexit from these drivers.

Based on patches originally written by Bill Pemberton, but redone by me
in order to handle some of the coding style issues better, by hand.

Remove all #inclusions of asm/system.h preparatory to splitting and killing
it.  Performed with the following command:

Add check if palette register number is in correct range for few drivers
which miss it.  The regno value comes indirectly from user space.

Two drivers has converted check from BUG_ON() macro to just return an
error (non-zero value).

Inspection of real hardware has revealed one of the clock frequencies known
to be supported by PMAGB-B hardware is off by 3kHz.  Following is a fix.
