The @end parameter for all extent io tree helpers is inclusive, but
the call site in extent_fiemap() is passing an exclusive end into
btrfs_lock_extent(), which will step into the next block unexpectedly.

Pass the inclusive end into btrfs_lock_extent() and
btrfs_unlock_extent().

The nodesize and sectorsize are all u32 values, there is no need to use
u64 for local usage.

Furthermore some call sites also use "blocksize" or "bs" for sectorsize,
also change them to use the minimal type u32 instead.

We have support for optional string to be printed in ASSERT() (added in
19468a623a9109 ("btrfs: enhance ASSERT() to take optional format
string")), it's not yet everywhere it could be so add a few more files.

Try to finish what was left after 1c094e6ccead7a ("btrfs: make a few
more ASSERTs verbose").

This is the result of running the Coccinelle script from
scripts/coccinelle/api/kmalloc_objs.cocci. The script is designed to
avoid scalar types (which need careful case-by-case checking), and
instead replace kmalloc-family calls that allocate struct or union
object instances:

Single allocations:	kmalloc(sizeof(TYPE), ...)
are replaced with:	kmalloc_obj(TYPE, ...)

Array allocations:	kmalloc_array(COUNT, sizeof(TYPE), ...)
are replaced with:	kmalloc_objs(TYPE, COUNT, ...)

Flex array allocations:	kmalloc(struct_size(PTR, FAM, COUNT), ...)
are replaced with:	kmalloc_flex(*PTR, FAM, COUNT, ...)

(where TYPE may also be *VAR)

The resulting allocations no longer return "void *", instead returning
"TYPE *".

Instead of using a bare atomic, use the refcount_t type, which despite
being a structure that contains only an atomic, has an API that checks
for underflows and other hazards. This doesn't change the size of the
extent_buffer structure.

This removes the need to do things like this:

    WARN_ON(atomic_read(&amp;eb-&gt;refs) == 0);
    if (atomic_dec_and_test(&amp;eb-&gt;refs)) {
        (...)
    }

And do just:

    if (refcount_dec_and_test(&amp;eb-&gt;refs)) {
        (...)
    }

Since refcount_dec_and_test() already triggers a warning when we decrement
a ref count that has a value of 0 (or below zero).

This is an exported function so it should have a 'btrfs_' prefix by
convention, to make it clear it's btrfs specific and to avoid collisions
with functions from elsewhere in the kernel.

Rename the function to add 'btrfs_' prefix to it.

The most trivial pattern for the auto freeing when the variable is
declared with the macro and the final btrfs_free_path() is removed.
There are almost none goto -&gt; return conversions and there's no other
function cleanup.
