We can add const to many parameters, this is for clarity and minor
addition to safety. There are some minor effects, in the assembly
code and .ko measured on release config. This patch does not cover all
possible conversions.

Do a cleanup in the short headers:

- add forward declarations for types referenced by pointers
- add includes when types need them

This fixes potential compilation problems if the headers are reordered
or the missing includes are not provided indirectly.

Previous patch made sure that btrfs_setxattr_trans() is called only when
transaction NULL.  Clean up btrfs_setxattr_trans() and drop the
parameter.

Preparatory patch, as we are going split the calls with and without
transaction to use the respective btrfs_setxattr() and
btrfs_setxattr_trans() functions. Export btrfs_setxattr() for calls
outside of xattr.c.

The xattr_handler::get prototype returns int, use it. The only ssize_t
exception is the per-inode listxattr handler.
