fs: convert a pile of fsync routines to errseq_t based reporting

This patch converts most of the in-kernel filesystems that do writeback out of the pagecache to report errors using the errseq_t-based infrastructure that was recently added. This allows them to report errors once for each open file description. Most filesystems have a fairly straightforward fsync operation. They call filemap_write_and_wait_range to write back all of the data and wait on it, and then (sometimes) sync out the metadata. For those filesystems this is a straightforward conversion from calling filemap_write_and_wait_range in their fsync operation to calling file_write_and_wait_range. Add a helper function that clears buffer heads from a block device aliasing passed bh. Use this helper function from filesystems instead of the original unmap_underlying_metadata() to save some boiler plate code and also have a better name for the functionalily since it is not unmapping anything for a *long* time. The kiocb already has the new position, so use that. The only interesting case is AIO, where we currently don't bother updating ki_pos. We're about to free the kiocb after we're done, so we might as well update it to make everyone's life simpler. While we're at it also return the bytes written argument passed in if we were successful so that the boilerplate error switch code in the callers can go away.
