headers: Remove pagemap.h from swap.h
swap.h does not need pagemap.h itself, and only three files were relying
on pagemap.h being implicitly included by swap.h.
This does not significantly reduce the number of files depending on
pagemap.h, but it does reduce the entanglement of our headers a little.
