Pull kthread vfs updates from Christian Brauner:
"This stops kernel threads from sharing filesystem state with
userspace. This work is about 3 cycles old and has been in -next
for about that time.
When the kernel boots init_task creates PID 1 and then kthreadd. From
that point every kthread and PID 1 share the same fs_struct. That is
why pivot_root() has to rewrite the fs_struct of all kthreads. The
rewriting exists so that kthreads can use init's filesystem state when
they want to. It also means userspace can move the ground out from
under the kernel.
PID 1 now gets a completely separate fs_struct. All kthreads are
anchored in a private SB_KERNMOUNT instance of nullfs that cannot be
mounted on and cannot be used to follow other mounts. Userspace init
can no longer affect kthread filesystem state and kthreads can no
longer affect userspace fs state without explicit opting in to that.
Path lookup from a kthread now fails by default. It makes it
deliberately hard to offload security sensitive operations into init's
filesystem state from a kthread.
Places that legitimately need to look something up there opt in
through the new scoped_with_init_fs() which temporarily overrides the
caller's fs_struct with init's. usermodehelpers remain the only kernel
tasks that genuinely share init's filesystem state, since they execute
random binaries in the root filesystem (excellent...).
The visible result is that /proc/2/root is a nullfs with an empty
mountinfo while /proc/1/root is the real root"

tcm_usbg_drop_nexus() permits session removal once tpg_port_count
reaches zero. However, usbg_port_unlink() currently decrements that
count from the fabric_pre_unlink() callback, before core_dev_del_lun()
waits for active se_lun references to drain.
If removal of the last LUN races a nexus removal, the latter can observe
a zero port count and call target_remove_session(). This frees
sess_cmd_map while an in-flight struct usbg_cmd, including its work item,
can still be accessed.
Overlapping the last-LUN unlink with nexus removal reproduces this
lifetime violation as a DEBUG_OBJECTS "free active" warning for
usbg_cmd_work, followed by a target-core BUG/Oops.
The generic target-core unlink path has no callback after
core_dev_del_lun() completes. Add an optional fabric_post_unlink()
callback and use it for the f_tcm port count. The count now remains
nonzero until core_dev_del_lun() has finished draining active LUN
references, preventing nexus removal from freeing the session during
command completion.

When target_get_sess_cmd() fails during session shutdown because
percpu_ref_tryget_live() returns false, the command keeps the
se_cmd->cmd_cnt pointer that __target_init_cmd() assigned earlier
without owning a reference. Final release through
target_release_cmd_kref() then issues an unmatched percpu_ref_put().
Commit 8e288be8606a ("scsi: target: Pass in cmd counter to use during
cmd setup") moved the cmd_cnt assignment ahead of the reference
acquisition. Clear se_cmd->cmd_cnt whenever the initial
target_get_sess_cmd() fails in target_init_cmd() and
target_submit_tmr(), so release performs exactly one matching put per
acquired reference.

core_scsi3_emulate_pro_register_and_move() maps the PERSISTENT RESERVE OUT
parameter list with transport_kmap_data_sg() and parses the destination
TransportID with target_parse_pr_out_transport_id(). For an iSCSI
TransportID (FORMAT CODE 01b), iscsi_parse_pr_out_transport_id() returns
the ISID in iport_ptr as a raw pointer into that mapped buffer.
The function then unmaps the buffer with transport_kunmap_data_sg() before
dereferencing iport_ptr in strcmp(), __core_scsi3_locate_pr_reg() and
core_scsi3_alloc_registration(). When the parameter list spans more than
one page (PARAMETER LIST LENGTH > 4096), transport_kmap_data_sg() uses
vmap() and transport_kunmap_data_sg() does vunmap(), so the kernel virtual
address backing iport_ptr is torn down and every subsequent dereference is
a use-after-free read of the unmapped region.
Keep the parameter list mapped until iport_ptr is no longer needed: drop
the early transport_kunmap_data_sg() and unmap once on the success path,
right before returning. The error paths already unmap through the existing
"if (buf) transport_kunmap_data_sg(cmd)" at the out: label, which now runs
on every post-map error exit because buf is no longer cleared early. Only
reads of the mapping happen while spinlocks are held; the map and unmap
calls remain outside any lock. The sibling caller
core_scsi3_decode_spec_i_port() already uses the buffer before unmapping it
and is left unchanged.

core_scsi3_decode_spec_i_port() and core_scsi3_emulate_register_and_move()
hand the raw PERSISTENT RESERVE OUT parameter buffer to
target_parse_pr_out_transport_id() without telling it how many bytes are
valid. For an iSCSI TransportID (FORMAT CODE 01b),
iscsi_parse_pr_out_transport_id() locates the ",i,0x" ISID separator with
an unbounded strstr() (and on the error path prints the name with a further
unbounded "%s"). An initiator can submit a TransportID whose iSCSI name
contains neither a ",i,0x" substring nor a NUL terminator, filling the
parameter list to its end, so the scan runs off the end of the buffer.
When the parameter list spans more than one page the buffer is a multi-page
vmap (transport_kmap_data_sg()), so the over-read walks into the trailing
vmalloc guard page and oopses (KASAN: vmalloc-out-of-bounds in strstr). It
is reachable by any fabric that delivers a PR OUT to a device exported
through an iSCSI TPG, including a guest via vhost-scsi.
Pass the number of received bytes down to the parser and validate the iSCSI
TransportID's own self-described length (ADDITIONAL LENGTH + 4) once, up
front: reject it if it is below the spc4r17 minimum or larger than the
received buffer, then bound the separator search, the ISID walk and the
name copy by that length. This is the length check the callers already
perform after the parse (core_scsi3_decode_spec_i_port() compares tid_len
against tpdl, core_scsi3_emulate_register_and_move() validates it against
data_length), moved ahead of the scan. Also drop the unbounded "%s" of the
unterminated name.
Add per-format explicit name-length checks before copying into i_str,
rather than silently truncating with min_t: for FORMAT CODE 00b reject if
the descriptor body (tid_len - 4 bytes) cannot fit in
i_str[TRANSPORT_IQN_LEN]; for FORMAT CODE 01b reject if the name portion
(from &buf[4] up to the separator) cannot fit. Both checks make the bounds
intent explicit at each format branch.
While here, also reject a FORMAT CODE 01b TransportID whose ",i,0x"
separator sits at the very end of the descriptor: that leaves an empty ISID
and points the returned port nexus pointer at buf + tid_len, one past the
descriptor, which the registration code (__core_scsi3_locate_pr_reg(),
__core_scsi3_alloc_registration()) then dereferences as the ISID string --
the same over-read of the parameter buffer for a malformed descriptor.

tcm_loop_target_reset is supposed to handle all the LUNs on a target but
it's only doing a TMR_LUN_RESET so only that one LUN is handled. This
will cause us to return early while IOs to other LUNs are still hung in
lower layers. This just removes the target reset handler for the driver
because LIO doesn't support target resets and for the common case where
this is run from the scsi-ml error hamdler we have already tried an
abort and lun reset so waiting again is most likely useless.

A constant-time memory comparison is more suitable than plain memcmp()

A mutual CHAP handshake requires target processing of an initiator-sent
CHAP_I identifier. The RFC 3720 specification states:
11.1.4. Challenge Handshake Authentication Protocol (CHAP)
...
CHAP_A=<A> CHAP_I=<I> CHAP_C=<C>
...
Where N, (A,A1,A2), I, C, and R are (correspondingly) the Name,
Algorithm, Identifier, Challenge, and Response as defined in
[RFC1994], N is a text string, A,A1,A2, and I are numbers
CHAP_I parsing currently calls extract_param(), which returns the
@identifier string (stripped of any 0b/0B or 0x/0X prefix) and a @type
which indicates DECIMAL, HEX, or BASE64 encoding (based on any stripped
prefix).
Any HEX encoded CHAP_I string is further processed via:
ret = kstrtoul(&identifier[2], 0, &id);
This is incorrect for two reasons:
* The @identifier string has already been stripped of the 0x/0X prefix,
so skipping the first two bytes omits part of the number.
* The kstrtoul() call specifies a base of 0, which will see
&identifier[2] parsed as a decimal, unless a '0x' or (octal) '0' is
erroneously present at that offset.
Fix this by passing the (zero-offset) identifier string to kstrtoul()
along with a base=16 parameter. Also add an explicit error handler for
BASE64 encoding.
Hex-encoded CHAP_I handling can be testing using the libiscsi EncodedI
test linked below.

chap_server_compute_hash() allocates client_digest as
kzalloc(chap->digest_size) and then, for BASE64-encoded responses,
passes chap_r directly to chap_base64_decode() without checking whether
the input length could produce more than digest_size bytes of output.
chap_base64_decode() writes to the destination unconditionally as long
as there is input to consume. With MAX_RESPONSE_LENGTH set to 128 and
the "0b" prefix stripped by extract_param(), up to 127 base64 characters
can reach the decoder. 127 characters decode to 95 bytes. For SHA-256
(digest_size=32) this overflows client_digest by 63 bytes; for MD5
(digest_size=16) the overflow is 79 bytes.
The length check at line 344 fires after the write has already happened.
The HEX branch in the same switch statement already validates the length
up front. Apply the same approach to the BASE64 branch: strip trailing
base64 padding characters, then reject any input whose data length
exceeds DIV_ROUND_UP(digest_size * 4, 3) before calling the decoder.
Stripping trailing '=' before the comparison handles both padded and
unpadded encodings. chap_base64_decode() already returns early on '=',
so the full original string is still passed to the decoder unchanged.
The mutual CHAP path decodes CHAP_C into initiatorchg_binhex, which is
kzalloc(CHAP_CHALLENGE_STR_LEN). extract_param() caps initiatorchg at
CHAP_CHALLENGE_STR_LEN characters, so at most CHAP_CHALLENGE_STR_LEN-1
base64 characters reach the decoder. The maximum decoded size,
DIV_ROUND_UP((CHAP_CHALLENGE_STR_LEN-1) * 3, 4), is less than
CHAP_CHALLENGE_STR_LEN, so no overflow is possible there. A comment is
added at the call site to document this.

iscsi_encode_text_output() concatenates "key=value\0" records into
login->rsp_buf, an 8192-byte kzalloc(MAX_KEY_VALUE_PAIRS) buffer
allocated in iscsit_alloc_login_setup_buffer(). The three sprintf() call
sites in this function (lines 1398, 1411, 1424 in v7.1-rc2) never check
the remaining buffer capacity:
*length += sprintf(output_buf, "%s=%s", er->key, er->value);
*length += 1;
output_buf = textbuf + *length;
The 8192-byte ceiling at iscsi_target_check_login_request() bounds the
*input* Login PDU payload, but a single PDU can carry up to 2048 minimal
four-byte "a=b\0" pairs, each unknown key expanding to a 16-byte
"a=NotUnderstood\0" output record via iscsi_add_notunderstood_response().
2048 * 16 = 32 KiB of output into an 8 KiB buffer, producing a ~24 KiB
heap overrun in the kmalloc-8k slab.
The fix introduces a static iscsi_encode_text_record() helper that uses
snprintf() with a per-call bounds check against the remaining buffer,
and threads a u32 textbuf_size parameter through
iscsi_encode_text_output(). Both call sites in
iscsi_target_handle_csg_zero() (PHASE_SECURITY) and
iscsi_target_handle_csg_one() (PHASE_OPERATIONAL) pass
MAX_KEY_VALUE_PAIRS. On overflow the encoder logs the condition, calls
iscsi_release_extra_responses() to drop queued records, and returns -1;
both caller sites now emit ISCSI_STATUS_CLS_INITIATOR_ERR /
ISCSI_LOGIN_STATUS_INIT_ERR via iscsit_tx_login_rsp() before returning,
so the initiator sees an explicit failed-login response rather than a
silent connection drop. (Prior to this patch only the PHASE_OPERATIONAL
caller did that; the PHASE_SECURITY caller is converted to the same
shape.)

iscsit_handle_text_cmd()
Two latent bugs in the Text-phase handler, both present since the
original LIO integration in commit e48354ce078c ("iscsi-target: Add
iSCSI fabric support for target v4.1"):
1) DataDigest CRC buffer overread (4 bytes past text_in).
text_in is kzalloc()'d at ALIGN(payload_length, 4). rx_size is then
incremented by ISCSI_CRC_LEN to make room for the received DataDigest
in the iovec, but the same (now-bumped) rx_size is passed as the
buffer length to iscsit_crc_buf():
if (conn->conn_ops->DataDigest) {
...
rx_size += ISCSI_CRC_LEN;
}
...
if (conn->conn_ops->DataDigest) {
data_crc = iscsit_crc_buf(text_in, rx_size, 0, NULL);
iscsit_crc_buf() walks rx_size bytes of text_in with crc32c(), so
when DataDigest is negotiated it reads 4 bytes past the end of the
text_in allocation. KASAN reproduces this directly on the unpatched
mainline tree as slab-out-of-bounds in crc32c() called from the Text
PDU path. The OOB bytes feed crc32c() and are then compared against
the initiator-supplied checksum, so the value does not flow back to
the attacker, but the kernel does read past the buffer on every Text
PDU with DataDigest=CRC32C.
Fix by passing the actual padded payload length
(ALIGN(payload_length, 4)) that was used for the kzalloc().
2) Stale cmd->text_in_ptr re-free (double-free) on ERL>0 bad DataDigest
drop.
On DataDigest mismatch with ErrorRecoveryLevel > 0 the handler
silently drops the PDU and lets the initiator plug the CmdSN gap:
kfree(text_in);
return 0;
cmd->text_in_ptr still points at the freed buffer. The next Text
Request on the same ITT re-enters iscsit_setup_text_cmd(), which
unconditionally does
kfree(cmd->text_in_ptr);
cmd->text_in_ptr = NULL;
freeing the same pointer a second time. Session teardown via
iscsit_release_cmd() has the same shape and hits the same double-free
if the connection is dropped before a second Text Request arrives.
On an unmodified mainline tree the bug-1 CRC overread fires first on
the initial valid Text Request and perturbs the subsequent state, so
#4 was isolated by building a kernel with only the bug-1 hunk of this
patch applied plus temporary printk() observability around the three
relevant kfree() sites. The observability prints are not part of
this patch. On that build, a three-PDU Text Request sequence after
login produces two back-to-back splats:
BUG: KASAN: double-free in iscsit_setup_text_cmd+0x??
BUG: KASAN: double-free in iscsit_release_cmd+0x??
showing the same pointer freed in the ERL>0 drop path and again in
iscsit_setup_text_cmd() (next Text Request on the same ITT) and once
more in iscsit_release_cmd() (session teardown). On distro kernels
with CONFIG_SLAB_FREELIST_HARDENED=y (default) the double-free
becomes a remote kernel BUG(); on non-hardened kernels it corrupts
the slab freelist.
Fix by clearing cmd->text_in_ptr after the kfree() in the ERL>0 drop
path. With both hunks applied #4 is directly observable on the stock
tree without observability printks; fixing bug-1 alone would mask #4
less, not more, so the hunks are submitted together.
Both fixes are one-liners. The Text PDU state machine is unchanged and
the wire protocol is unaffected.

The TCM_LOOP LUN creation process calls device_register() to create the
device, which in turn invokes tcm_loop_driver_probe() registered with
the TCM_LOOP bus to create and register the scsi_host. However, if the
scsi_host memory allocation fails or scsi_add_host() fails, the
device_register() process still returns success. Subsequently, when the
user binds the LUN to a specific backend device, it accesses the NULL or
freed scsi_host.

This issue is fixed by:
1. Setting the tcm_loop_hba's scsi_host to NULL, if scsi_add_host()
fails.
2. Checking the tcm_loop_hba's scsi_host after device_register().
3. Checking the tcm_loop_hba's scsi_host in tcm_loop_driver_remove().

If ecdb_ahdr->ahslength is zero, two bugs follow:
kmalloc(be16_to_cpu(ecdb_ahdr->ahslength) + 15, ...)
allocates 15 bytes, but the immediately following memcpy writes
ISCSI_CDB_SIZE (16) bytes into it, a one-byte heap overflow. Also:
memcpy(cdb + ISCSI_CDB_SIZE, ecdb_ahdr->ecdb,
be16_to_cpu(ecdb_ahdr->ahslength) - 1);
(u16)0 - 1 promotes to (int)-1 which converts to SIZE_MAX as size_t,
causing a massive out-of-bounds write.
Reject ahslength == 0 with ISCSI_REASON_PROTOCOL_ERROR before the kmalloc.
Also reject ahslength values that exceed the actual AHS buffer advertised.

target_tg_pt_gp_members_show() formats LUN paths with snprintf() into a
256-byte stack buffer, then will memcpy() cur_len bytes from that
buffer. snprintf() returns the length the output would have had, which
can exceed the buffer size when the fabric WWN is long because iSCSI IQN
names can be up to 223 bytes. The check at the memcpy() site only
guards the destination page write, not the source read, so memcpy() will
read past the stack buffer and copy adjacent stack contents to the sysfs
reader, which when CONFIG_FORTIFY_SOURCE is enabled, fortify_panic()
will be triggered.
Commit 27e06650a5ea ("scsi: target: target_core_configfs: Add length
check to avoid buffer overflow") added the same bound to the
target_lu_gp_members_show() but the tg_pt_gp variant was missed so
resolve that here.

The PERSISTENT RESERVE OUT command's PREEMPT service action provides two
different functions: 1. preempting persistent reservations and 2.
removing registrations. In the latter case the spec says:
b) ignore the contents of the SCOPE field and the TYPE field;
The code currently validates the SCOPE and TYPE fields even when PREEMPT
is called to remove registrations.
This patch achieves spec compliance by validating the SCOPE and TYPE
fields only when they will actually be used.
To confirm my interpretation of the specification I tested against HPE
3PAR storage and found the TYPE field is indeed ignored in this case.

The f_op->mmap interface is deprecated, so update uio_info to use its
successor, mmap_prepare.
Therefore, replace the uio_info->mmap hook with a new
uio_info->mmap_prepare hook, and update its one user, target_core_user,
to both specify this new mmap_prepare hook and also to use the new
vm_ops->mapped() hook to continue to maintain a correct udev->kref
refcount.
Then update uio_mmap() to utilise the mmap_prepare compatibility layer to
invoke this callback from the uio mmap invocation.

Replace the deprecated[1] strncpy() with strscpy() in
transport_dump_vpd_proto_id(), transport_dump_vpd_assoc(),
transport_dump_vpd_ident_type(), and transport_dump_vpd_ident().
All four functions follow the same pattern: a local
buf[VPD_TMP_BUF_SIZE] (254 bytes) is zeroed with memset(), populated
via sprintf()/snprintf() (always NUL-terminated), then conditionally
copied to p_buf with strncpy(). The p_buf destination is used as a
C string by all callers in target_core_configfs.c: strlen(buf) and
sprintf(page+len, "%s", buf) to build sysfs output.
NUL-padding is not required: callers in target_core_configfs.c
pre-zero p_buf with memset() or initializer before calling these
functions, and consume p_buf only as a NUL-terminated C string via
strlen() and "%s", never exposing trailing bytes.
No behavioral change: the source buf is always NUL-terminated and
shorter than VPD_TMP_BUF_SIZE, so strscpy() produces identical output.

The target_core_file doesn't initialize the aio_cmd->iocb for the
ki_write_stream. When a write command fd_execute_rw_aio() is executed,
we may get a bogus ki_write_stream value, causing unintended write
failure status when checking iocb->ki_write_stream > max_write_streams
in the block device.
Let's just use kzalloc_flex when allocating the aio_cmd and let
ki_write_stream=0 to fix this issue.

tcm_loop_target_reset() violates the SCSI EH contract: it returns SUCCESS
without draining any in-flight commands. The SCSI EH documentation
(scsi_eh.rst) requires that when a reset handler returns SUCCESS the driver
has made lower layers "forget about timed out scmds" and is ready for new
commands. Every other SCSI LLD (virtio_scsi, mpt3sas, ipr, scsi_debug,
mpi3mr) enforces this by draining or completing outstanding commands before
returning SUCCESS.
Because tcm_loop_target_reset() doesn't drain, the SCSI EH reuses in-flight
scsi_cmnd structures for recovery commands (e.g. TUR) while the target core
still has async completion work queued for the old se_cmd. The memset in
queuecommand zeroes se_lun and lun_ref_active, causing
transport_lun_remove_cmd() to skip its percpu_ref_put(). The leaked LUN
reference prevents transport_clear_lun_ref() from completing, hanging
configfs LUN unlink forever in D-state:
INFO: task rm:264 blocked for more than 122 seconds.
rm D 0 264 258 0x00004000

Fix this by making tcm_loop_target_reset() actually drain commands:
1. Issue TMR_LUN_RESET via tcm_loop_issue_tmr() to drain all commands that
the target core knows about (those not yet CMD_T_COMPLETE).
2. Use blk_mq_tagset_busy_iter() to iterate all started requests and
flush_work() on each se_cmd — this drains any deferred completion work
for commands that already had CMD_T_COMPLETE set before the TMR (which
the TMR skips via __target_check_io_state()). This is the same pattern
used by mpi3mr, scsi_debug, and libsas to drain outstanding commands
during reset.

This allows userspace to request if we complete in the backend context
or the frontend driver. It works the same as submission where you can
write 0 to 2 to complete_type:
0 - Use the fabric driver's preference.
1 - Complete from the backend calling context if the fabric supports
it.
2 - Queue the completion to LIO's completion workqueue.

To complete a command several drivers just drop their reference and add
it to list to be processed by a driver specific thread. So there's no
need to go from backend context to the LIO thread then to the driver's
thread. When avoiding the LIO thread, IOPS can increase from 20-30% for
workloads like:
fio --filename=/dev/sdb --direct=1 --rw=randrw --bs=8K \
--ioengine=libaio --iodepth=128 --numjobs=$jobs
where increasing jobs increases the performance improvement (this is
using NVMe drives with LIO's submit_type=1 to directly submit).
Add the infrastructure so drivers and userspace can control how to
complete a command like is done for the submission path. In this commit
there is no behavior change and we continue to defer to the LIO
workqueue thread. In the subsequent commits we will allow drivers to
report what they support and allow userspace to control the behavior.

In flush_write_buffer, &p->frag_sem is acquired and then the loaded store
function is called, which, here, is target_core_item_dbroot_store(). This
function called filp_open(), following which these functions were called
(in reverse order), according to the call trace:
down_read
__configfs_open_file
do_dentry_open
vfs_open
do_open
path_openat
do_filp_open
file_open_name
filp_open
target_core_item_dbroot_store
flush_write_buffer
configfs_write_iter
target_core_item_dbroot_store() tries to validate the new file path by
trying to open the file path provided to it; however, in this case, the bug
report shows:
db_root: not a directory: /sys/kernel/config/target/dbroot
indicating that the same configfs file was tried to be opened, on which it
is currently working on. Thus, it is trying to acquire frag_sem semaphore
of the same file of which it already holds the semaphore obtained in
flush_write_buffer(), leading to acquiring the semaphore in a nested manner
and a possibility of recursive locking.
Fix this by modifying target_core_item_dbroot_store() to use kern_path()
instead of filp_open() to avoid opening the file using filesystem-specific
function __configfs_open_file(), and further modifying it to make this fix
compatible.

This converts some of the visually simpler cases that have been split
over multiple lines. I only did the ones that are easy to verify the
resulting diff by having just that final GFP_KERNEL argument on the next
line.
Somebody should probably do a proper coccinelle script for this, but for
me the trivial script actually resulted in an assertion failure in the
middle of the script. I probably had made it a bit _too_ trivial.
So after fighting that far a while I decided to just do some of the
syntactically simpler cases with variations of the previous 'sed'
scripts.
The more syntactically complex multi-line cases would mostly really want
whitespace cleanup anyway.

This is the exact same thing as the 'alloc_obj()' version, only much
smaller because there are a lot fewer users of the *alloc_flex()
interface.
As with alloc_obj() version, this was done entirely with mindless brute
force, using the same script, except using 'flex' in the pattern rather
than 'objs*'.

This was done entirely with mindless brute force, using
git grep -l '\<k[vmz]*alloc_objs*(.*, GFP_KERNEL)' |
xargs sed -i 's/\(alloc_objs*(.*\), GFP_KERNEL)/\1)/'
to convert the new alloc_obj() users that had a simple GFP_KERNEL
argument to just drop that argument.
Note that due to the extreme simplicity of the scripting, any slightly
more complex cases spread over multiple lines would not be triggered:
they definitely exist, but this covers the vast bulk of the cases, and
the resulting diff is also then easier to check automatically.
For the same reason the 'flex' versions will be done as a separate
conversion.

This is the result of running the Coccinelle script from
scripts/coccinelle/api/kmalloc_objs.cocci. The script is designed to
avoid scalar types (which need careful case-by-case checking), and
instead replace kmalloc-family calls that allocate struct or union
object instances:
Single allocations: kmalloc(sizeof(TYPE), ...)
are replaced with: kmalloc_obj(TYPE, ...)
Array allocations: kmalloc_array(COUNT, sizeof(TYPE), ...)
are replaced with: kmalloc_objs(TYPE, COUNT, ...)
Flex array allocations: kmalloc(struct_size(PTR, FAM, COUNT), ...)
are replaced with: kmalloc_flex(*PTR, FAM, COUNT, ...)
(where TYPE may also be *VAR)
The resulting allocations no longer return "void *", instead returning
"TYPE *".

The code in sbp_make_tpg() limits "tpgt" to UINT_MAX but the data type of
"tpg->tport_tpgt" is u16. This causes a type truncation issue.
When a user creates a TPG via configfs mkdir, for example:
mkdir /sys/kernel/config/target/sbp/<wwn>/tpgt_70000
The value 70000 passes the "tpgt > UINT_MAX" check since 70000 is far less
than 4294967295. However, when assigned to the u16 field tpg->tport_tpgt,
the value is silently truncated to 4464 (70000 & 0xFFFF). This causes the
value the user specified to differ from what is actually stored, leading to
confusion and potential unexpected behavior.
Fix this by changing the type of "tpgt" to u16 and using kstrtou16() which
will properly reject values outside the u16 range.

In clang version 21.1 and later the -Wimplicit-enum-enum-cast warning
option has been introduced. This warning is enabled by default and can
be used to catch .queuecommand() implementations that return another
value than 0 or one of the SCSI_MLQUEUE_* constants. Hence this patch
that changes the return type of the .queuecommand() implementations from
'int' into 'enum scsi_qc_status'. No functionality has been changed.

Remove <linux/hex.h> from <linux/kernel.h> and update all users/callers of
hex.h interfaces to directly #include <linux/hex.h> as part of the process
of putting kernel.h on a diet.
Removing hex.h from kernel.h means that 36K C source files don't have to
pay the price of parsing hex.h for the roughly 120 C source files that
need it.
This change has been build-tested with allmodconfig on most ARCHes. Also,
all users/callers of <linux/hex.h> in the entire source tree have been
updated if needed (if not already #included).

Add a third parameter 'const struct io_comp_batch *' to the rq_end_io_fn
callback signature. This allows end_io handlers to access the completion
batch context when requests are completed via blk_mq_end_request_batch().
The io_comp_batch is passed from blk_mq_end_request_batch(), while NULL
is passed from __blk_mq_end_request() and blk_mq_put_rq_ref() which don't
have batch context.
This infrastructure change enables drivers to detect whether they're
being called from a batched completion path (like iopoll) and access
additional context stored in the io_comp_batch.

In iscsit_dec_session_usage_count(), the function calls complete() while
holding the sess->session_usage_lock. Similar to the connection usage count
logic, the waiter signaled by complete() (e.g., in the session release
path) may wake up and free the iscsit_session structure immediately.
This creates a race condition where the current thread may attempt to
execute spin_unlock_bh() on a session structure that has already been
deallocated, resulting in a KASAN slab-use-after-free.
To resolve this, release the session_usage_lock before calling complete()
to ensure all dereferences of the sess pointer are finished before the
waiter is allowed to proceed with deallocation.

In iscsit_dec_conn_usage_count(), the function calls complete() while
holding the conn->conn_usage_lock. As soon as complete() is invoked, the
waiter (such as iscsit_close_connection()) may wake up and proceed to free
the iscsit_conn structure.
If the waiter frees the memory before the current thread reaches
spin_unlock_bh(), it results in a KASAN slab-use-after-free as the function
attempts to release a lock within the already-freed connection structure.
Fix this by releasing the spinlock before calling complete().
